  • Respite Care: Taking time for yourself is crucial. Our respite care services provide short-term relief so that you can rest, recharge, and care for your own health while knowing your loved one is in capable hands.

New to GUIDE?

Guiding an Improved Dementia Experience (GUIDE) is a new Medicare program designed to offer enhanced care coordination and support for individuals living with dementia, as well as the caregivers who support them. GUIDE provides personalized care, resources, and tools to help manage the complexities of dementia, ensuring that both patients and their caregivers receive the comprehensive support they need.

If you’re a caregiver supporting a loved one through their dementia journey and assisting with daily tasks like bathing, dressing, managing medications, or handling finances, the GUIDE program could offer valuable support. Learn more and get the help you deserve today.

Already enrolled in GUIDE?

If your loved one is already a GUIDE participant, please contact your Care Navigator to:

  • Participate in our virtual support groups: Connect with other caregivers who understand your challenges and can offer advice, empathy, and solidarity.
  • Utilize our 24/7 support line: Access round-the-clock support from professionals who can provide guidance, answer questions, and offer reassurance whenever caregiving becomes difficult or overwhelming. You’re never alone, no matter the time of day.
  • Coordinate respite care: Learn about services that provide temporary relief for caregivers so you can take care of your own well-being.
